12/5/2023 EnchantaVaughn Approved 1.Behavior: Josiah appeared to be in a positive mood while discussing with QMHP-C how his day was going. He reported that his evening was going good. QMHP-C engaged mother in a discussion to determine behaviors since the last session. Mother reports Josiah continues to struggle with increasing his communication skills. 2. Behavior: Josiah appeared to be withdrawn as he continues to be upset with not learning how to communicate effectively. 1.Intervention: QMHP-C processed with Josiah and family on positive expressive comments. QMHP-C has Josiah to verbalize positive feelings and utilized the feeling faces to support Josiah with recognizing his feelings and thoughts. QMHP-C modeled appropriate behaviors for Josiah to exhibit respectful behaviors toward his mother and siblings. 2. Intervention: QMHP-C utilized role play for Josiah to be able to identify and process someone who displays good communication skills. QMHP-C conducted discussion for Josiah to identify words that allow him to demonstrate good communication skills. QMHP-C requested that Josiah think of words that may be controversial and challenged Josiah to decrease his frequent usage of negative words. QMHP-C taught communication skills and being assertive. 1. Response: Josiah appeared to be receptive to QMHP-C’s interventions and services that were provided to him during the session. Mother reported that it is difficult at times to talk to Josiah as he struggles with actively listening and being defiant toward limits certain rules and etc. being displayed in the home and community setting. 2. Response: Josiah reported that he has close relational ties with his mother however, Josiah reported that he gets fused at a lot by his mother which he becomes reactive by displaying negative behaviors. Josiah reported that he tries to talk and converse with his mother but when angry it has been difficult.
